Membership Information
*Our practice does not bill insurance. Your only bill is the monthly membership fee which covers unlimited visits and communication with your doctor. There are no additional co-pays or unexpected charges. Like all primary care clinics, Haden Family Medicine can still refer and order testing for patients who have medical needs outside the scope of this practice.*
Prices: ​
*One time $75/household charged at time of registration.
Children 0-18: $25/month with a participating member enrollment
*$50/month for first child enrolled without participating member
Age 19-39: $50/month
Age 40-64: $75/month
Age 65+: $100/month
Employer sponsored membership: $60/employee per month
